To be eligible:

            Have to have a Medicare card and be eligible for Part A.

            Have had a qualifying 3 night stay in the hospital.

            Be admitted to our facility within 30 days of that hospital stay.

 

*Our Medicare skilled services include:

            24 hour Nursing care

            Physical Therapy, Occupational Therapy, and Speech Therapy

            Room and Board

            Lab Expenses

            Transportation

            Doctor Visits

            Medical Supplies.

            Drugs and Medications

 

*Burwell CMHC provides the same benefits as “Swing bed” in the hospital for a longer period, because we are a skilled facility.
